This Agreement confirms that the undersigned Parent or Legal Guardian (referred to as "Releasor") grants permission to [the Best Halloween Store Ever Inc. and the Shoppes at Westlake Village] (referred to as "Photographer") for the use of photographs taken of the minor child(ren) named in this completed form.

II. Grant of Rights and Usage

The Releasor grants the Photographer the irrevocable and perpetual right, license, and permission to use the photographs and/or video recordings of the minor child(ren) for commercial and non-commercial purposes, in all forms of media, including but not limited to:

1. Advertising, Marketing, and Promotion (e.g., promotional materials, brochures, advertisements).

2. Website and Social Media (e.g., portfolio, Facebook, Instagram).

3. Publications (e.g., magazines, books).

4. Display (e.g., at exhibitions, events, or in the studio).

The Releasor authorizes the Photographer to edit, crop, alter, or composite the photographs without restriction. The Photographer retains the copyright to all photographs.

III. Waiver and Release

The Releasor hereby waives the right to inspect or approve the finished product or the use of the images. The Releasor releases and forever discharges the Photographer from any and all claims and demands arising out of or in connection with the use of the photographs, including any claims for libel, defamation, invasion of privacy, or right of publicity.

The Releasor understands and agrees that no monetary compensation will be paid for the execution of this release or for the use of the photographs.
